Job Description:
A company is looking for a Care Manager, Telephonic Nurse 1.
Key Responsibilities
Assess and evaluate members' needs to promote optimal wellness
Identify and resolve barriers to effective care while monitoring
patient progress
Utilize various strategies to manage members' physical,
environmental, and psycho-social health issues
Required Qualifications
Must reside in Puerto Rico
Must be fully bilingual (English/Spanish)
2+ years of relevant experience
Licensed Practical Nurse (LPN) in Puerto Rico or equivalent with no
disciplinary action
Comprehensive knowledge of Microsoft Office applications
